This Consent is made in connection with behind-the-wheel driving instruction services provided by Go & Drive, P.C. (“Go & Drive”) to the student identified below.

I am the parent or legal guardian of the above-named student (the “Student”). I hereby authorize Go & Drive and its authorized driving instructors to pick up the Student from the residence listed and provided in the student details for scheduled behind-the-wheel driving lessons, even if I am not present at the time of pickup.

Electronic Signature Acknowledgment

By checking the box and clicking “Submit” (or “I Agree”), I am signing this Consent electronically. I understand and agree that my electronic signature is legally binding and has the same force and effect as a handwritten signature. I consent to conduct this transaction and receive this disclosure electronically, and I represent that I am the parent or legal guardian of the student named in this registration and have the authority to grant this authorization.

  1. The Student has my permission to leave the residence and enter the instructor’s vehicle without my being present.
  2. I am solely responsible for ensuring that the Student is ready for pickup at the scheduled time and location.
  3. Go & Drive and its instructors are not required to make personal contact with me prior to each lesson.
  4. Go & Drive’s responsibility for the Student is limited to the period beginning when the Student enters the instructor’s vehicle for the scheduled lesson and ending when the Student exits the vehicle at the authorized drop-off location.
  5. Any request to change or revoke this authorization, including any change to the approved pickup address, must be submitted to Go & Drive in advance and in writing, and such change will not take effect until acknowledged by Go & Drive.
  6. I represent and warrant that I am the Student’s parent or legal guardian and possess the legal authority to grant this authorization.
  7. This authorization shall remain in full force and effect for all driving lessons provided by Go & Drive unless and until I revoke it in writing.
