This Acknowledgment and Consent is entered into in connection with the enrollment of the Student for driving instruction services provided by Go & Drive, P.C.

The undersigned acknowledges and agrees as follows:

  1. Contracting Party. My enrollment agreement for driving instruction services is with Go & Drive, P.C. (“Go & Drive”).
  2. Subcontracted Driving Instruction. I understand and agree that Go & Drive may, in its discretion, subcontract all or a portion of the behind-the-wheel driving instruction services to its affiliated company, Go & Ride, LLC. (“Go & Ride”). If Go & Drive elects to do so, the driving lessons may be scheduled, administered, and conducted by Go & Ride and its authorized instructors on behalf of Go & Drive.
  3. Sharing of Student Information. I authorize Go & Drive to disclose, transfer, and share with Go &Ride any information, records, documents, and files relating to the Student or the Student’s enrollment that are reasonably necessary for scheduling, administering, providing, documenting, or completing the driving instruction services.
  4. Use of Student Records. I understand that Go & Ride may receive, maintain, update, and return Student records to Go & Drive as necessary to provide the contracted driving instruction services and to comply with applicable legal and regulatory requirements.
  5. No Additional Consent Required. I acknowledge that Go & Drive may engage Go & Ride to provide driving instruction services without obtaining additional consent from me for each lesson or assignment.
  6. No Change to Enrollment. I understand that subcontracting driving instruction services to Go & Ride does not change my enrollment in the driving instruction program or the terms of my agreement with Go & Drive, except that the driving instruction may be performed by Go & Ride on Go & Drive’s behalf.
